John Waters does not usually play supporting medical professionals. In American Horror Story: 13 he does, and the character is named Mr. Phibes.

The casting is not subtle. Waters has spent decades joking that he has always wanted Vincent Price’s career. Ryan Murphy handed him the closest possible approximation: a clinical, black-humored medical specialist extraordinaire whose name is lifted straight from Price’s 1971 cult classic The Abominable Dr. Phibes.

In the early episodes Phibes works alongside Constance Langdon on the thirteenth floor of the Apollyon, overseeing the care or the testing of the ancient, ailing Mr. Nas. He is already one of the season’s most distinctive new faces.

The Vincent Price Connection

In the original film, Dr. Anton Phibes is a disfigured, revenge-obsessed organist and scientist who kills according to the ten plagues of Egypt. Waters’ version is less operatic and more clinical, but the DNA is unmistakable. The name is the same. The medical authority is the same. The sense that something is deeply wrong with the procedures he performs is the same.

Waters himself leaned into the comparison when the casting was announced. Murphy clearly designed the part as a deliberate gift to a cult filmmaker who has always belonged in this universe.

What Phibes Actually Does on Screen

In the premiere stretch Phibes appears during the medical routines surrounding Mr. Nas. He collects vials, specifically thirteen of them in one sequence. He moves through the penthouse with the calm of someone who has done this work for a very long time.

His presence reframes the hospice arrangement. Ben is the new caregiver. Constance is the handler. Phibes is the specialist who seems to understand the true nature of the patient far better than either of them.

Is American Horror Story Season 13 worth watching?

Yes for longtime fans. The season reunites characters from Murder House, Coven, Cult, Freak Show and more in a single story centered on the number 13 and the Apollyon.

Why John Waters Belongs in This Universe

John Waters has spent a career making audiences sit with bad taste, sacred monsters, and jokes that hurt. American Horror Story has always borrowed that energy even when Waters himself was offscreen. Putting him in the Apollyon as Mr. Phibes is less a stunt cameo and more a correction. The franchise is finally employing one of its spiritual relatives.

The Vincent Price homage sharpens the casting. Price’s Phibes was theatrical revenge and plague logic. Waters’ Phibes is clinical, dry, and present for the routines around Mr. Nas. The name creates expectation. The performance undercuts it into something colder. That gap is where the character lives.

Thirteen vials in a medical sequence is the bridge back to the season’s central math. Phibes is not only comic relief. He is another professional who understands that the number is operational. Constance manages the social floor. Phibes manages the bodily floor. Ben is the newcomer learning both.

For viewers who know Waters’s films, watch his stillness. He does not need big gestures to dominate a scene. In a season crowded with returning icons, that restraint may be the most disruptive choice of all.

Medical Horror Without the Usual Gore Lecture

Phibes could have been written as a torture specialist. Instead he is written as staff. That choice is colder. A torturer is an event. A staff member is a system. When Phibes collects thirteen vials, the horror is not only the needle. It is the professionalism. Someone has to do this every cycle. Someone has to treat Nas as a patient and as a project at the same time.

Waters understands how to play that professionalism for laughs without draining the threat. The audience is allowed to smile and still feel that the thirteenth floor is not a safe workplace. That balance is rarer than pure shock, and it is why the character already feels permanent in a season full of temporary cameos.

Phibes and the Sound of the Thirteenth Floor

Listen to how quiet Phibes can make a room. In a season full of declarations about destiny and thirteen, his low-key authority is a different instrument. He does not need to announce the theme. He performs it with inventory. That inventory — vials, procedures, proximity to Nas — may eventually matter as much as any monologue about portals.

As more of Nas’s care becomes public inside the story, Phibes will either remain a controlled presence or tip into something closer to Price’s original plague logic. Either path works for Waters. The important part is that Season 13 already trusts him with the medical heart of the thirteenth floor. That trust is the real casting statement.

When the October episodes return to the thirteenth floor, watch Phibes as closely as you watch Nas. The patient is the mystery. The specialist may be the method.
