Something shifts inside the Apollyon on the night of the plane crash. On the 13th floor, an 87-year-old man named Mr. Nas begins to change. His body contorts. He vomits a thick black liquid. Horns push through the skin of his skull. For viewers who have followed American Horror Story for more than a decade, this is not just body horror. It is the first clear signal that the season’s “ultimate evil” has already arrived.
Ryan Murphy spent years teasing a threat that sits above the Antichrist. After three episodes, the evidence points straight at the frail figure in the penthouse. The transformation is timed to mass death. It is not subtle. And it is not finished.
What Exactly Happens During the Transformation
In the second episode, titled “1:13 AM,” Ben DeSoto is alone with his new patient for the overnight shift. During a routine sponge bath he notices details that do not belong on a human body: a tail, claw-like hands, and what look like the beginnings of cloven feet. Later that same night the real change begins.
Mr. Nas’s body twists in ways no medical condition explains. He heaves up a viscous black substance that one character later compares to crude oil. Small horns become visible on his head. The sequence is short, but it is the most visceral image the season has delivered so far.
The same black liquid appears earlier, at the exact moment the plane carrying Ben and his girlfriend Kira breaks apart mid-air. Twelve people die. Ben survives in seat 13F. On the 13th floor of the Apollyon, Mr. Nas is already expelling the black fluid as the bodies fall.
Episode 2 repeats the pattern. Twisty the Clown walks into a Los Angeles clown school and murders exactly 13 people, including the teacher played by Tig Notaro. Back in New York, Mr. Nas vomits the black liquid again. The show is not asking viewers to notice a coincidence. It is stating a causal link.
Constance’s Explanations Do Not Hold
Jessica Lange’s Constance Langdon is the daytime caregiver. When Ben reports the black liquid and the non-human features, she offers medical explanations that sound rehearsed. Lewy body dementia. An old ulcer that never healed. Medication side effects. Stress from “the kind of career Mr. Nas has had.”
None of it accounts for the horns, the tail, or the perfect timing with distant massacres. Constance also insists that Mr. Nas is loved by many and should be praised for “what he has done for the world.” The language is closer to worship than caregiving. By the end of the third episode she has already begun gathering 13 “lucky leading ladies” for a Rauhnacht celebration that ends in the basement portal known as Club Hell.
The previous overnight caregiver, Rosalind Myers, died by suicide. Ben learns this only after he has already accepted the job. The information lands like a quiet warning the show never fully explains.
Why the Transformation Matters More Than the Horns
American Horror Story has always mixed the grotesque with the symbolic. The horns and the black vomit are effective scare tactics, but the deeper point is the connection to death on a precise numerical scale. Every major body count so far lands on the number 13. Every time the count is complete, Mr. Nas reacts physically.
Ryan Murphy described the season’s central threat as something that sits “above the Antichrist.” The Antichrist storyline already played out in Apocalypse. This new figure appears older, more rooted, and already feeding. The Apollyon itself is revealed in episode 3 as a portal to Hell. Mr. Nas lives on the top floor of that building. The vertical design is deliberate: the source above, the gate below, and Ben trapped in the middle without understanding the architecture of the place he now works.
Paul Anthony Kelly is listed as Young Nas. The existence of a younger version suggests the season will eventually show how this entity has moved through time. For now the older form, played by Fedor Steer, is the one undergoing the visible change. Steer spends most of his screen time in bed beneath a painting of the Last Supper that contains exactly 13 figures. The visual is not accidental.
What Viewers Should Watch for Next
The next three episodes drop on October 1 under the titles “Is That All There Is?,” “Date Night,” and “The Return of Devil’s Night.” If the pattern holds, another cluster of 13 deaths will trigger another physical reaction from Mr. Nas. The horns may grow. The black liquid may change. Or the transformation may complete and force Constance to drop the medical cover stories entirely.
Until then the central question remains open but increasingly narrow. Mr. Nas is not a hospice patient who happens to have unusual features. He is the reason the features appear when people die in groups of 13. The black vomit is not an ulcer. It is the visible cost of whatever he is doing to the world outside the Apollyon.
American Horror Story Season 13 has only just begun to show its hand. The transformation of Mr. Nas is the first clear card on the table.

Frequently Asked Questions
What happens to Mr. Nas’s body in American Horror Story Season 13?
In episode 2 his body contorts, he vomits a thick black liquid, and horns become visible on his head. The same black substance appears during the plane crash in episode 1 and again during Twisty’s clown-school massacre.
Is Mr. Nas the ultimate evil Ryan Murphy talked about?
Murphy said the season’s threat sits above the Antichrist. The timing of Mr. Nas’s physical reactions with every major death count of 13, combined with Constance’s reverential language and the Apollyon’s status as a Hell portal, makes him the strongest candidate after three episodes.
Who plays Mr. Nas?
Fedor Steer plays the elderly version. Paul Anthony Kelly is credited as Young Nas, indicating the character exists across different time periods.
Why does Constance protect Mr. Nas?
She offers medical excuses for his symptoms and speaks of him in almost religious terms, calling him a mastermind and urging Ben to praise “what he has done for the world.” Her loyalty appears deeper than ordinary caregiving.
Does the black liquid connect to the plane crash and the clown massacre?
Yes. The show cuts directly from the deaths to Mr. Nas expelling the black substance in both cases. The pattern is presented as deliberate rather than coincidental.
